To deliver optimal services and ensure patients access the necessary treatment, completion of the intake process is essential. This protocol is designed to evaluate our ability to provide comprehensive care. Before filling out our online intake form you must be a resident of Michigan, Illinois, or California and review the steps outlined below.

Kindly note that submitting an intake form does not automatically guarantee services from our office.
